    The goal of the Anticoagulation Clinic is to optimize the patient’s anticoagulation therapy with warfarin, low molecular weight heparin or heparin therapy. Patients are provided comprehensive education on medication compliance and safety, importance of laboratory monitoring, and dietary and lifestyle impacts of anticoagulation therapy. Patients are carefully monitored using established evidence based protocols. Therapy management occurs following lab results, via face to face visits with patients and in collaboration with the patient’s physician.
